How does composite breast augmentation work?

Before the procedure

Before the operation, an initial consultation with Dr. Favre is arranged to determine whether you are a candidate for composite breast augmentation. We take the time to listen to you, examine you, and advise you on the most appropriate solutions for you. The consultation combines a consultation for breast augmentation with implants and liposuction.
We will review your medical history and your specific needs. To select the implants, we use different sizes and shapes so you can visualize the desired result using a special bra. The size and shape of the breast implants are discussed and chosen based on your preferences and Dr. Favre's recommendations. The areas where the fat will be harvested and injected will also be discussed.
A preoperative assessment including an ultrasound and/or mammogram and a blood test (to check for anemia or coagulation disorders) is standard. It is strongly recommended that you stop smoking three weeks before and after the procedure to reduce the risk of infection.

The procedure

On the day of the procedure, composite breast augmentation is performed in two stages:
We begin the procedure by liposuctioning the desired areas (abdomen, thighs, back, etc.) and collecting the fat in a closed circuit using our EVL technique.
The breast implants are then inserted under the mammary glands or under the pectoral muscle (we use the dual plane technique when placing the implant under the muscle) to increase the volume of the breasts. The breast implants are inserted using the “No Touch” technique with the “Funnel Keller” implant device, leaving only a small scar: Minimal Scar. We do not use drains for this procedure.
After the breast implants have been inserted, fat is then added to the breasts to increase their volume, improve their shape, and reduce the outline of the implants. This fat transfer gives a more natural look and feel.

After the procedure

The procedure is performed on an outpatient basis, which means that you can leave the Bellefontaine clinic on the same day as the procedure. The final result will be visible three months after the breast augmentation, but you will notice physical and psychological changes from the very first day. Close follow-up is provided with clear instructions and medication included in the package (prophylactic antibiotics and painkillers to be taken for seven days). When the implant is placed under the pectoral muscle, more pain will be felt for the first 4 days and will subside over the first two weeks. It is important not to sleep on your stomach for 8 weeks after a composite breast augmentation. Your weight should not vary significantly after the procedure to prevent significant changes in breast volume.

General information

A composite breast augmentation is a breast surgery procedure that combines breast implants (we mostly use Motiva Ergonomix implants, but we also use anatomical implants) with liposuction to take your own fat and transfer it to the breast. This process is called lipofilling.
The advantage of combining implants and fat is that it gives the breasts an even more natural appearance by hiding the implant even more under a layer of fat. Fat transfer allows for softer contours and hides the outline of the implants. The breasts also feel more natural and supple to the touch than without the addition of fat. This difference is not very noticeable in patients who already have a good B cup size before the procedure.
It is also common after weight loss or pregnancy to experience a loss of volume in the upper part of the breast, also known as “breast sagging,” which can be effectively corrected with lipofilling. This gives the breasts a more harmonious appearance and corrects the drooping effect by filling the upper part of the breast. However, adding fat alone does not create a “rounded” effect on the upper part of the breast.

What are the advantages of composite breast augmentation compared to breast augmentation with implants alone or with fat alone?

The advantage is that you can choose from all available implant sizes and shapes (Motiva, round, or anatomical), which is not possible when using fat alone. The advantage of fat injection is that it hides the implant and improves the shape and texture of the breast while


What are the disadvantages of composite breast augmentation?

There are disadvantages associated with implants: higher risk of infection, capsular contracture, displacement, etc. There are also risks associated with fat injection, as the rate of resorption cannot be known in advance, and the patient must have sufficient fat reserves. Weight should be stable. This procedure is contraindicated in patients with a close family member who has had breast cancer.

Some other important points

Post-operative care: The dressing is changed after one week at the Bellefontaine Clinic, and a Marena® sports bra must be worn continuously for 8 weeks to support the breasts and promote healing. You can take your first shower one week after the procedure using a waterproof dressing.

Return to work: Most patients with office jobs can return to work after 3 to 4 days. For more manual activities such as hairdressing or physiotherapy, a 3-week break is recommended.

Scar treatment: From the third week onwards, special scar creams (Kelimed) and massages are recommended to optimize their appearance. Wearing silicone dressings (Mepiform) for 3 to 6 months is also recommended. Protect your scars from the sun while they are pink or red using SPF 50+ sunscreen.

Risks and side effects

Like any surgical procedure, composite breast augmentation carries certain general risks associated with any surgery, such as infection or hematoma.

Specific complications include:

  • Implants: Capsular contracture, displacement, double bubble.
  • Lipofilling: Partial resorption of the injected fat, asymmetry, or fat necrosis (rare).

The latest generation of Motiva implants are durable and safe, requiring replacement only in the event of complications. Furthermore, by using only smooth implants, the risk of ALCL (anaplastic large cell lymphoma – a rare tumor linked to textured implants) has never been demonstrated.
